The Nurse Practitioner is responsible for the provision and management of quality primary health care to individuals. Your work will be within a family practice setting and Administrative staff. Operating within your full scope of practice as a Nurse Practitioner in this full-time, permanent position, you will work collaboratively with all staff members in treating identified illnesses, promoting patient health and preventing disease.
QUALIFICATIONS:
- 3 years of experience working in a primary care setting
- NP Certificate from accredited program
- Current registration with the College of Nurses of ontario as a Nurse Practitioner- Primary Health Care
- Excellent health assessment and clinical skills
- CPR certification
- Working knowledge of computer systems and applications
- Excellent verbal and written communication skills
- Demonstrates the ability to work well with diverse ethno-racial and cultural groups
In this role, you will:
- Conduct periodic examinations and risk assessments that may include care of at-risk populations, wellness exams, pap tests and breast exams, paediatric care, well-baby visits, immunizations and some acute care
- Provide care to patients with specific intermittent illnesses
- Collaborate with other multidisciplinary team members and external partners
- Develop and conduct a broad range of health promotion and chronic disease management processes, programs, and patient education (group and individual) in collaboration with multidisciplinary team members
- Maintain accurate, concise and confidential EMR documentation pertaining to patient history and encounter data
- Complete necessary FHT Program business documentation
- Communicate ideas and concerns to the team and participate in problem solving
